
The
Diabetes Management Center offers evaluation, individual and group
education and support for people living with type one and type two
diabetes. The center strives to empower patients to successfully
manage their diabetes on a daily basis.
Because successful diabetes management depends largely on the
patient, education is extremely important. Patients learn about
the disease itself, their prescribed medication, diet, nutrition
and how to recognize and treat hypoglycemia (low blood sugar)
and hyperglycemia (high blood sugar). Patients also learn techniques
for motivation setting personal goals.
Diabetes Education
The Diabetes Management Center offers Diabetes Dynamics, a two-class
course that teaches the basics of diabetes management, nutrition,
foot care, exercise, goal setting and motivation. Both day and

Wound Care
The Wound Center provides multidisciplinary outpatient treatment
for patients with chronic non-healing wounds. These wounds are
usually the result of diabetes, excessive pressure (bedsores),
circulatory conditions, trauma or surgery. The patient's initial
visit includes a comprehensive evaluation, which is used by staff
to develop an individualized care plan using the most advanced
treatments available. Education is provided throughout the course
of treatment to promote rapid healing and prevent recurrence.
Patients are referred to the Wound Center by home health personnel
or physicians, or they may be self-referred.
Support
Diabetes Support Group
Sponsored by the Diabetes Management Center, this group meets
on the last Thursday of each month from 1 to 2 p.m. Members share
information and support each other to promote successful diabetes
