Care managers are social workers who help guide patients through the continuum of care from the time of admission until discharge or transfer. They facilitate communication between the members of the patient's care team (physicians, nurses, therapists, etc.) and act as a link between the clinical and financial aspects of patient care.

Services provided by care managers include:

  • Discharge planning - determining whether the patient will be discharged home or to a nursing or rehab facility and what services and equipment he or she will need
  • Acting as a liaison with outside agencies or facilities
  • Ensuring the patient receives the appropriate level of care
  • Advocacy with insurance companies
  • Ensuring that the patient's financial resources are used appropriately
  • Determining the best options for the patient both clinically and financially
  • Assisting patients and their families with compliance with physician recommendations
  • Assisting patients and families with decision making
  • Helping to prevent readmission by ensuring the patient has access to all necessary resources and services
